Resident Care Coordinator?Position Summary:?

?The Resident Care Coordinator works as a liaison between residents, resident families, and staff. The Resident Care Coordinator?s duties also include problem-solving resident concerns and coordinating care with the Caregivers. The Resident Care Coordinator may be an RN or LVN when necessary per State Regulations.?

?Responsibilities of the Resident Care Coordinator:

--- Assisting with the Caregiver scheduling and resident assignments, working within the department?allowances.
--- Coordinate admissions with assistant administrators including supervising move-ins to be sure?accommodations are as desired and care is immediately implemented.
--- Complete family/resident admission interviews.
--- Immediately bring prohibited conditions or at-risk residents in need of a higher level of care to the?attention of the assistant administrator.
--- Coordinate care planning with home health agencies on-site, working within community policy.
--- Arrange for transportation as desired by the resident.
--- Arrange for resident special needs involving other departments, verifying follow through.
--- Function as a liaison with families ensuring special needs/requests/complaints are addressed.
--- Inform assistant administrator, in writing and verbally, of all family or resident complaints.
--- Read all communication notes regarding the community between the Caregiver shifts.
--- Assist with the coordinate staff training and in-service schedules with the Assistant Administrator
--- Assist with the supervision of the medication room and orders, working with Medication Aides and?Caregivers.
--- Other duties as assigned.?

Education and Experience:?

--- A.A. Degree in Health Care or related field from an accredited college preferred
--- may require education to obtain state nursing license (LVN/LPN or RN) per State Regulations
--- a minimum or three year?s relevant experience.
--- Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment.?
