Job Number: 202473Department: Social ServicesHours: 7:00p-7:30aIn this busy Social Worker position you will work in our ED to provide social work interventions for Emergency Department patients and their families, and assist in other areas of the hospital as needed after hours.Perform psychiatric and substance abuse assessments to determine a safe discharge plan. Document assessment and interventions according to standard work. Participate in safety planning and de-escalation in the ED.Provide care coordination with outside agencies to ensure continuity of care. Coordinate with ITA Court and testifies as needed, including writing affidavits when making a referral to the designated mental health professional.Perform psychosocial assessments for patients to determine appropriate disposition from the ED, coordinating transportation and providing additional care planning resources. Assist in care planning about and education on the appropriate use of the ED for patients who are high utilizers of emergency services.Provide short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability. Help optimize care transitions for patients who receive their outpatient care at VMMC.Participate in quality assurance and utilization review to enhance patient satisfaction, and participate as a member of an interdisciplinary team to develop a coordinated care plan for patients.Participate in informal and formal consultations; provide, present, and participate in support and training; arrange speakers and develop materials and programs.Participate in Medical Center committees and special projects as assigned.With numerous awards and distinctions, including our recognition as Top Hospital of the Decade by The Leapfrog Group, Virginia Mason has transformed health care through our innovations in patient safety and our ability to provide patients in our community with easier access to our high-quality care.
