Job Summary
The Licensed Mental Health Counselor (LMHC) is responsible for providing comprehensive assessment, case management, consultation and treatment for a wide range of behavioral health issues in a community health center setting. LMHCs support behavioral health care provided by primary care providers, in consultation with a psychiatric specialist and/or other clinical staff as appropriate.

Essential Duties and Responsibilities
Support s and closely coordinate s mental health care with the patient's primary care provider and other treating mental health providers.
Screen s and assess es patients for common mental and substance use disorders.
Provide s patient education about common mental and substance use disorders and available treatment options.
Monitor s patents (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Support s psychotropic medication management prescribed by primary care provider, focusing on treatment adherence, attention to side effects, and effectiveness of treatment.
Provide s brief counseling or psychotherapy using evidence-based techniques such as behavioral activation, motivational interviewing, or other relevant skills.
Provide s or facilitate s in-clinic or outside referrals to evidence-based psychosocial treatments as clinically indicated.
Conduct s regularly scheduled (usually weekly) caseload consultation with program director , c ommunicating resulting treatment recommendations to the patient's p rimary c are p rovider ; t hese consultations will primarily focus on patients who are new to treatment or who are not improving as expected.
Facilitate s patient engagement and follow-up in mental health care.
Track s patient follow-up and clinical outcomes using the web-based care management tracking system (MHITS).
Document s in person and telephone encounters in MHITS and use s the system to identify and reengage patients who may be lost to follow up.
Document s patient progress and treatment recommendations in MHITS, in order to facilitate information sharing with primary care providers and other treatment providers.
Facilitate s treatment plan changes for patients who are not improving as expected in consultation with the primary care providers and psychiatric specialist. These may include changes in medications or psychosocial treatments or appropriate referrals for additional services.
Facilitate s referrals for clinically indicated services outside the primary care clinic (e.g., social services as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Facilitate s Social Security Disability eligibility process with community service office.
Complete s relapse prevention plans with patients who are in remission.
Participate s in regular training and technical assistance activities for m ental h ealth c oordinators.
Responsible to learn about and implement trauma-informed practices, as a key component to wellness, within the scope of work for this position.
Abides by Health Insurance Portability and Accountability Act of 1996 in reference to all confidential information obtained through job tasks.

Licensed in the State of Washington as a Licensed Mental Health Counselor (required)
CPR Certification (required)
Valid driver's license (required)

Supervisory Responsibilities
This position does not include supervisory responsibilities.

Work Environment and Physical Demands
This job operates in a clinical and/or outreach setting.
Required to talk, hear, see, and sit/stand.
Frequently required to move around the facility.
Use hands to finger, handle or feel; reach with hands and arms .
Climb or balance and stoop, kneel , crouch or crawl.
Frequently required to lift and/or move up to 25 pounds.
May be exposed to mechanical parts and moderate noise levels.
