Work includes intake/discharge planning activities, initiating/monitoring the quality management program, timely and accurate completion of patient and billing documentation, and coordinating interdisciplinary teams that include healthcare practitioners, rehabilitation therapists, social workers, and other healthcare disciplines or providers depending upon individual patient assessments.Must be able to perform all physical job duties accurately in a fast-paced environment with frequent interuptions.Essential Duties:Delivers and coordinates care to patients with alterations in skin/tissue integrity within the scope of practice and under the direction of physician or other clinician.Manages and coordinates patient case/wound management in accordance with established standards, strictly defined timelines, and overall plan of care.Completes all required documentation in a timely, accurate, and thorough manner in accordance with FHS standards and procedures.Initiates and monitors the Quality Management program for the Wound Care Center.QualificationsJob Requirements:Associate's degree in nursing, BSN preferredMinimum two years of related work experience working with an interdisciplinary clinical team in one or more of the following clinical areas: Med/Surg, Gerontology, Home Health, or Wound Care that demonstrates attainment of the requisite job knowledge skills/abilities.Current Washington State Registered Nurse licensure required.Current American Heart Association Healthcare Provider BLS Certification required.Current professional certification (e.g. WCC, CWON, CWS or equivalent) is preferred.
