UW Medical Center - Montlake has an outstanding opportunity for a Registered Nurse/VAD Coordinator to join our Heart Transplant team. VAD Coordinators are specialized nurses, who practice both in the inpatient and outpatient care setting that facilitate the care and education of Ventricular Assist Device (VAD) patients and their families from initial contact, during the evaluation process and the perioperative period, ensuring continuity of care across the advanced heart failure VAD/heart transplant continuum.

VAD Coordinators also perform patient and caregiver education, provide educational/ outreach programs regarding VAD therapy to health care providers, patient groups, community organizations, and others in the UW Medicine service area. The VAD Coordinator is an integral member of the larger interdisciplinary team contributing to excellence in patient care, research, teaching, and in providing leadership to the organization.

SCHEDULE

  • Full-time, Day shift.
  • Position requires VAD Coordinator to be on-call to provide MCS program coverage.

    RESPONSIBILITIES
    Clinical/Care Coordination:
  • Participate in the evaluation process of patients who are being worked-up for VAD/heart transplant to assess their candidacy.
  • Appropriately coordinates testing required to complete the work-up prior to implant.
  • Performs assessment of patients with VADs to establish, implement and evaluate individualized plan of care.
  • Provides timely and appropriate clinical advice and interventions through direct patient contact as well as telephone contact.
  • Arranges all appropriate medical therapy and testing under the guidance of physicians and advance practice providers.
  • Reviews and evaluates lab results and notifies the physician or advance practice provider of abnormalities.
  • Appropriately documents findings and interventions in the patient's medical record and transplant database.
  • Attends VAD Clinic.
  • Participate in discharge process including assessment of readiness for discharge.
  • Ensure the patient's home environment is safe and secure; provide documentation to providers, EMS department, local hospital and electric company at discharge.
  • Ensure that the patient is able to receive VAD dressing supplies to appropriately care for their VAD driveline exit site.
  • Participates in the VAD implantation process.
  • Manages VAD equipment and accurate tracks VAD inventory.

    Education/Outreach:
  • Takes the opportunity with every contact to educate patients and caregivers about their VAD, medications, signs and symptoms of infection and other device complications.
  • Provide pre and post VAD teaching to the patient and their caregivers and document in the medical record.
  • Coordinates and develops professional education programs/seminars regarding VAD patients for the physicians, staff, and medical specialty units caring for UW VAD patients.

    Research/Data:
  • Provide pre and post VAD teaching to the patient and their caregivers and documents in the electronic medical record.
  • Coordinates clinical research protocols and serves as primary coordinator for INTERMACS, the national database for Mechanical Circulatory Support.
  • Maintains all case report forms, patient research records and complies with all Institutional Review Board (IRB), study sponsor, and Food and Drug (FDA) requirements in order to conduct clinical research.
  • Documents patient specific data for tracking in the transplant database.

    Program Maintenance:
  • Functions as a liaison for the VAD Program in interaction with numerous internal and external organizations and staff (These include but are not limited to, referring physicians, case managers, other transplant center personnel, community and government agencies).
  • Assists the organization in achieving and maintaining Joint Commission certification as a VAD center for destination therapy.

    Quality:
  • Participates in the VAD Program Quality Improvement Process.
  • Plays a significant role in developing, reviewing and evaluating effectiveness of policies and procedures within the program.

    Equipment:
  • Track patient-assigned VAD equipment in the UW database ordering system at the time of implantation and throughout the care continuum.
  • Manage the process of any returned or damaged equipment that needs to be reordered or re-furbished.
  • Maintain close working relationship with Biomedical Engineering.

    REQUIREMENTS
  • Bachelor's Degree in Nursing from an accredited college.
  • Basic Life Support (BLS) for Health Care Providers issued by the American Heart Association required.
  • Clinical experience in a cardiovascular ICU, intermediate ICU, cardiology, or cardiac surgery setting preferred.
  • Three years of overall RN experience preferred.
  • Position requires VAD Coordinator to be on-call to provide MCS program coverage.

    LEGAL REQUIREMENTS
    Washington State Registered Nurse (RN) license required.

    DESIRED
    One year VAD experience preferred.
