position is full-time (40 hours/week) Monday to Friday. Employees
are required to have flexibility to work any of our 8-hour shift
schedules during our normal business hours of 8:00am - 5:00pm. We
offer 2 to 4 weeks of paid training. Primary Responsibilities:
- Create a positive experience and relationship with the
member
- Proactively engage the member to manage their care
- Provide member education
- Support transitions of care
- Help to keep members compliant with their care plans
- Partner with care team (community, providers, internal
staff)
- Knowledge and continued learning of community cultures and
values
- May conduct member assessments if needed
- Comprehensive care management
- Care coordination
- Health promotion
- Transitional planning and follow-up
- Individual and family support
- Referral to relevant community and social support services
- Improve health and self - management of conditions
- Provide transition and personalized care to help
- Reduce the progress of chronic disease
- Reduce inappropriate emergency department utilization,

you may be interested in.Required Qualifications:
- Bachelor's Degree (or higher) in social work, psychology,
social services, human services, or behavioral sciences OR
Associate's Degree (or higher) WITH Medical Assistant Certification
in the state of WA or National Certification OR a High School
Diploma / GED (or higher) WITH either an Indian Health Service
(IHS), an LPN license in the state of WA or equivalent work
experience as a Community Health Worker (CHW)
- Reside within a commutable distance of Pierce County, WA
- Resided within the local community for 1+ years
- Knowledge of culture and values in the local community
- Familiarity with the resources available within the
community
- Experience with Microsoft Excel (ability to create
spreadsheets, input data entry), Microsoft Word (creating and
editing documents) and Microsoft Outlook (send/receive emails)
- Access to reliable transportation that will enable you to
travel to client and / or patient sites within a designated
area
- Ability to travel locally within the county up to 60% of the
time with mileage reimbursement (once cleared due to Covid-19)
- Full COVID-19 vaccination is an essential job function of this
role. Candidates located in states that mandate COVID-19 booster
doses must also comply with those state requirements. UnitedHealth

well as all client requirements and will obtain necessary proof of
vaccination, and boosters when applicable, prior to employment to

Qualifications:
- Field Based experience
- Bilingual fluency in English and Spanish
- Experience working in Managed Care
- Knowledge of WA Medicaid and Medicare populations
- Experience in Counseling or Behavioral Health
- Case management experience
- Experience working with members of all ages (children to
