Nurse (LPN) - Posting #24064Hourly Rate: $29.81Position Summary:Sea
Mar has opened -an inpatient geriatric psychiatric program in
Seattle in the South Park area. We are seeking an -On-call
-Licensed Practical Nurse. Preferred is someone with experience in
working with the Mental Health/Behavioral -Health population
including knowledge of treatment planning, care plan and Mental
Health Progress Notes. Summary of job duties is -listed below.The
LPN must demonstrate the clinical knowledge and skills required to
provide quality patient/client care and services to all facility
patients. The incumbent must be knowledgeable about Behavioral
Health/Residential Treatment Facility Rules and Regulations to
provide care appropriate to the age of clients. Demonstrate
knowledge of the principles of growth and development over the
course of life, and apply this knowledge in the course of providing
excellent care.The LPN is responsible for providing excellent
nursing care to all patients as ordered by each patient's health
care provider, whether primary or specialist and to ensure that all
orders are being carried out as ordered. The LPN will work with the
patient/client and other members of the interdisciplinary care team
in the planning and delivery of nursing interventions, and client
care and services. To accomplish these objectives, excellent
critical thinking skills and teamwork are required.Essential Duties
and ResponsibilitiesThe LPN will:
- Work closely with the Patient/Client, the Client's Provider and
other members of the interdisciplinary team in coordinating care
and services.
- Provide direct care and services to Patients/Clients as per
organizational protocol, i.e. - insulin monitoring and injections,
etc.
- Complete a monthly summary progress note in the Client's
medical record after performing a head to toe assessment on the
Client
- Participate in the annual and/or change of condition
reassessment of residents as per Nurse Manager's request.
- Update the resident's service/care plan as needed and after a
hospitalization, or a change of condition, to include special
precaution such as behavioral plans, refusals of medication, crisis
interventions and so forth.
- Insure timely nursing and provider-ordered interventions
- Initiate, maintain and discontinue alert charting for residents
as per organizational protocol, developing and implementing
interventions as appropriate to meet the resident needs.
- Give/receive shift report to/from in-coming LN and to CNAs and
Med Techs on shift
- Supervise the on-floor work of CNAs and Med Techs to insure
quality care and services to all Residents, at all times.
- Consistently practice Universal precautions and sterile
technique, as required.
- Demonstrate knowledge of quarantine/isolation in cases of
infectious disease, with clear knowledge of routes of infection and
barriers appropriate to each one.
- Follow organizational protocol in handling emergency
situations.
- Demonstrate competency with administering injections, passing
medications and doing treatments, both with the physical action and
with the time management aspect of medication administration.
- Demonstrate a clear understanding of the rationale for checking
fasting blood sugars, administering insulin injections and
performing food choice counseling for diabetic patients, as well as
a competency with acts of the same.
- Institute emergency nursing interventions as necessary; contact
provider, family, others as per protocol.
- Ensure that medication administration is documented according
to protocol.
- Ensure medication storage, maintenance, dispensation and waste
logs are maintained in accordance with Cannon House's policies and
procedures.
- Ensure quality controls are performed and documented as
required.
- Respond to resident/employee complaints, phone calls and emails
in a timely manner and inform DNS and WC as needed. .
- LPN is responsible to notify Nurse Manager, Administrator and
family member if there is evidence of abuse, neglect, suicidal
attempt, or other significant changes in resident's condition.
- Demonstrate knowledge, proficiency and compliance with
regulatory requirements, including, but not limited to; HIPPA, RTF
and BH regulations, Department of Health regulations, and
contractual obligations.
- Be open to changes in her/his schedule, if needed to cover the
facility's staffing needs, and perform other duties as
assignedQualificationsTo achieve success as a Behavioral Health
LPN, an individual must be able to perform each essential duty
satisfactory and always strive to exceed basic requirements.
- The person in this position must possess the understanding,
physical health, emotional stability and personality suited to
meeting the health, mental and emotional health, socio/economic and
ability needs of the Resident population the Geriatric Facility
serves.
- The LPN must possess an interest and ability to work with
Resident groups from across the life span.
- A basic understanding of managed health care and the mechanics
of DSHS is very useful.
- A desire to learn about health promotion and disease prevention
and treatment options is highly desirable.Language Skills
- LPN must have the ability to read and write in English with
excellent charting and documentation skills;
- He/she must possess good communication skills suitable to
communicate with a wide range of people in many situations, and be
able to interact sensitively and appropriately with people of
diverse backgrounds.
- Bilingual in English/Spanish preferred not required. -Education
and/or Experience:
- The Geriatric Facility LPN must be licensed to practice as an
LPN in the state of Washington, and has successfully completed the
state mandated HIV education.
- LPN must obtain continuing education credits to meet licensure
requirements.
- LPN must be licensed in the State of Washington to practice as
a licensed practical nurse. Mental Health experience is
preferred.
- He/she must have experience working with a range of patient age
groups and health concerns.
- This position must obtain Basic Life Support (BLS) CPR within
90 days of hire date and is required to maintain current BLS CPR
