Advanced Practice Provider - Home Based Primary Care - Seattle, WA This locum APP position supports adult patients living with chronic kidney disease (CKD), end-stage renal disease (ESRD), and related metabolic disorders. Providers deliver high-quality, in-home, value-based care using technology-enabled clinical tools, with the goal of improving outcomes, reducing hospitalizations, and supporting health equity across complex patient populations. Rates: $90-$95/hr Schedule Hybrid | Full-Time | Monday–Friday No call, no weekends 3–4 days of in-home patient visits 1–2 days remote for charting, admin, and occasional telehealth Some weeks may involve 5 full field days Travel required up to 1 hour per visit (patients are generally clustered) AI-supported scheduling to optimize visit routes Day-to-Day Responsibilities Conduct initial assessments, follow-ups, and hospital transition visits Perform hands-on physical assessments (full vitals, head-to-toe exams) Provide disease and medication education Manage medications and care plans See 4–5 patients/day (up to 8 rarely) Visit lengths: New patients: 90 minutes Follow-ups: 30–60 minutes Participate in POD rounding to discuss high-risk patients with the care team Document in Athena with visibility to care manager notes in Salesforce Qualifications Minimum 2 years' experience as an NP/PA Preferred backgrounds: home health, primary care, internal medicine Not a fit: urgent care, hospitalist, surgical, pediatric Experience with chronic disease management and care planning preferred Bilingual required Culture & Growth Rapidly growing, innovative care model Strong patient-first foundation Opportunities to advance across specialty competency tracks: Nephrology Palliative Cardiac Pulmonary Endocrine Raises awarded as each competency is mastered Equal Opportunity We are an equal opportunity employer and value diversity across our organization.
