Details - Home health setting serving a primarily island-based territory, including Whidbey Island and Oak Harbor, with occasional work in Skagit County and Snohomish County - Patient population is primarily geriatric - Average caseload of 5–7 patients per day - Productivity expectations include 6 visits per week and a minimum of 30 points per week - 13-week contract - Schedule is Monday–Friday, 8-hour day shifts (8:00 AM–5:00 PM) with no weekend or on-call requirements - EMR: HCHB (experience required) - OASIS experience required - Mileage reimbursed at the IRS standard rate - iPad provided for documentation purposes Requirements - Minimum 1 year of recent home health physical therapy experience required - Current BLS certification required - OASIS experience required - Proficiency with HCHB EMR required - Active PT license required if applicable - Must be comfortable traveling within a territory up to 60 miles from the hub/office - First-time travelers will not be considered for this position Additional Information - Therapist must be flexible with territory assignments across multiple counties - Timely documentation and consistent communication are expected - Scrubs in any color are required; jeans are not permitted - Local candidates are accepted and eligible for the travel rate - All time-off requests must be submitted at the time of initial consideration - Two professional supervisor references from within the past 2 years are required, along with a full 7-year work history with all gaps explained American Traveler Job ID P-685497.
