Description: Perm candidates from other facilities are eligible for
a travel contract if they have been separated for at least 3
months. 1 year of experience is required, and first-time travelers
are accepted. Travelers may be floated to other campuses if census
is low. Weekend coverage is required for a minimum of 2 weekend
days per month. Typically, this means working one Saturday and one
Sunday per month, but not on the same weekend. Candidates must have
prior hospital experience. This role focuses on care coordination
for neuro and cardiac patients. The clinician needs to have
inpatient case management experience. There is an interdisciplinary
support team available, including various services. Patient ratios
during the day are expected to be around 20. This position involves
discharge planning, needs assessment, and care coordination tasks.
