UW Medical Center is seeking highly motivated clinical pharmacists to join a team who provide pharmaceutical care for our pain, orthopedics and home infusion patient populations. Our pharmacists provide extensive pharmaceutical care for both inpatients and outpatients, are a vital part of the multidisciplinary patient care team, and work under a collaborative practice agreement with the director of the pain clinic. In addition to direct patient care responsibilities, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transition of care, precept pharmacy students and residents, and perform duties of Pharmacist 2. UW Medical Center Pharmacy has an outstanding opportunity for a FULL-TIME CLINICAL PHARMACIST - PAIN SERVICE (Pharmacist 3, Rotating/Variable Shifts).

Clinical Pharmacist - Pain Service Responsibilities Include:

Drug therapy monitoring, including, but not limited to:

Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions and drug allergies.

Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed and per collaborative practice agreement.

Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.

Provide drug information and education to medical teams, nursing, and dietician staff.

Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders at each visit.
Education and Research:

Provide primary preceptorship of pharmacy residents and student pharmacists.

Provide education to medical/surgical providers and students, and nursing staff.

Present at educational conferences to multidisciplinary staff, as requested.

Develop and provide nursing in-services.

Provide didactic teaching to student pharmacists at the UW School of Pharmacy, as requested.

Participate in collaborative research projects with healthcare professionals, pharmacy residents, and student pharmacists.

Submit unique work for publication in professional journals.
Practice Management:

Implement and maintain departmental policies and procedures related to the safe and effective use of medications.

Routinely evaluate current updates in service-specific literature.

Develop and maintain drug therapy monitoring systems for patients.

Establish and maintain efficient lines of communication between pharmacists, physicians, nurses, and other members of the healthcare team.

Participate in pharmacy and hospital committees as requested.

Participate in the development and implementation of quality assurance programs for pharmacy services, including, but not limited to, prevention of medication errors.

Perform drug utilization evaluation and cost-containment analyses, as needed.

Perform duties of a Pharmacist 2. REQUIREMENTS: State of Washington Pharmacist License and two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.

Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.

Experience in hospital pharmacy practice, pain management, and/or PGY2 residency is desirable.
