Hiring Fair - Housing Case Manager


Job Title: Clinical Support Specialist
Days Off: TBD (generally Friday / Saturday or Saturday / Sunday)
Pay Range: $23.77 - 26.25 per hour (6 step pay scale. Steps 1-4 are hourly. Steps 5-6 are salaried)
Location: Seattle (North Beacon Hill)
Hours: TBD (generally Office Day)

We are hiring four new case managers at Hobson Place, DESC's newest Permanent Supportive Housing. Hobson opened last fall for 85 formerly homeless adults with disabilities. Now it is expanding to 177 units, and will soon host an integrated health care clinic onsite. You can help welcome 92 vulnerable neighbors into their new home, and provide support in their journey.



DESC is firmly committed to the belief that the solution to homelessness is simple: housing. We offer housing to people before anything else, without insisting on abstinence or treatment compliance. Our approach has been recognized as a model for other communities and garnered significant academic study and praise.

MAJOR DUTIES AND RESPONSIBILITIES

Facilitate engagement of residents through creative, resourceful strategies that build trust and confidence; publicize service availability.
Develop an Integrated Service Plan (ISP) in collaboration with residents, case managers, and other staff, outlining goals and strategies to ensure housing success.
Identify and coordinate those case management elements that will be provided by case managers from outside provider organizations.
Maintain regular contact with residents and case managers from outside provider organizations to monitor progress toward goals identified in the ISP, and to ensure adequate provision of identified services.
Intervene in crises and assist with building rule enforcement and lease compliance as necessary.
Provide education and support for residents to ensure success as leaseholders.
Assist residents in the formation of interest groups or therapeutic support groups; facilitate when appropriate.
Organize outings and activities to improve resident skills in activities of daily living and to increase the level of meaningful activity in their lives.
Work with residents and other staff to identify needs for additional services, groups and activities. Work with staff, outside service providers and community resources to initiate and facilitate these services and activities.
Provide individual counseling and information and referral services to residents as needed.
Facilitate community meetings.
Participate in staff meetings and trainings.
Other duties as assigned.

MINIMUM QUALIFICATIONS

Ability to obtain an Agency Affiliated Counselor (AAC) license in WA soon after hire.
Knowledge of local homeless, mental health, and chemical dependency service systems.
Ability to communicate and work effectively with individuals from diverse backgrounds.
Ability to work effectively with clients displaying a wide range of behavior.
Commitment to the philosophy of cooperation and continuity across agency programs and of consideration and respect for clients.





EDUCATION / EXPERIENCE REQUIREMENTS:


Relevant Bachelor's degree in social work, psychology, or related behavioral science, or
Bachelor's degree in any field and 1 year of paid work experience in social services, or
Relevant associates degree and 1 year of relevant paid work experience, or
Any Associates degree and 1 year of highly relevant paid work experience, or
A combination of 1 year of highly relevant paid work experience and formal training in clinical and case management skills to demonstrate an ability to successfully perform the job.


PREFERRED QUALIFICATIONS:


Two years' experience working with homeless adults disabled by chronic alcoholism, mental illness and/or drug use.
Valid Washington State driver's license and insurable driving record.
