Nurse Practitioners hired into Traveler roles are eligible to receive a sign-on bonus OR a reduced sign-on bonus plus three years of student loan repayment assistance through our Student Loan Repayment (SLR) program.   This could mean up to an additional $40K in benefits to you as well as the opportunity to pay down your student loan faster! In this role, you will conduct in-home health assessments (average in-home visit is 45-60 minutes) on enrolled Medicare Advantage members, Community and State and several other plans. The Traveler Nurse Practitioner assignments will be defined based on current business needs. Primary Responsibilities: Travel 85% away from home (statewide - overnights) and 15% local Conduct in-home assessments on health plan members, including past medical history, review of symptoms, physical examination, medication review and depression screening Responsible for checking vitals, conducting a physical exam that includes monofilament test, urine dipstick and foot exam (as appropriate) Identify diagnoses to be used in care management and active medical management Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment Communicate findings in your assessment that will be used to inform the PCP of potential gaps in care Educate members on topics such as disease process, medication and compliance Identify referral needs Identify urgent and emergent situations for proper intervention Comply with all HIPAA regulations and maintain security of protected health information (PHI) Required Qualifications: Nurse Practitioner Certification from either the ANCC or AANP as Adult (ANP), Family (FNP), Geriatric (GNP), or Adult Gerontology-Primary Care (AG-PCNP) Active, unencumbered state license to practice nursing in Washington, free from any open / unresolved disciplinary actions including probation or restrictions against privilege to practice Minimum of 1+ year of clinical experience in highest level of education OR 3+ years of experience as a Registered Nurse Ability to obtain DEA licensure / prescriptive authority post-hire in states where applicable Must have the ability to travel to member’s home / assigned location Must be able to travel by public transportation when necessary (eg. Trains, Airplanes, Subways, Ferry, etc.) Ability to lift a 30 pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations Ability to sit, stand and kneel as needed to perform physical assessment Ability to spend at least 1 hour with a member in their home, which may be in understaffed or remote areas, in the presence of pets or individuals who are tobacco users Proficient with electronic medical records;  computer literate and able to navigate the internet Since this position involves in-home visits, the ability to work autonomously Preferred Qualifications: 3+ years of experience in Gerontology, Cardiology, Internal Medicine or Endocrinology Previous Home care or home visit experience Previous Nursing Home experience Excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families Detailed oriented; dependable and reliable As a Fortune 6 company with over 325,000 people all pulling in the same direction, we’re used to setting the pace for change.
