Come join our team!Job Summary:This job is responsible for coordinating and overseeing daily activities and services of the CHI Franciscan Bariatrics program for the assigned facility in accordance with professional standards, business objectives and applicable regulatory/accreditation requirements. An incumbent assesses and monitors continuity of care and coordinates patient navigation/progress throughout the continuum of care, from point of first interest or referral, through the qualification period, the surgery process, post-op care, and post discharge to achieve desired quality outcomes. An incumbent works collaboratively with the Program Manager and key stakeholders to achieve center of excellence status and in ongoing program growth and maintenance.Work includes: 1) serving as liaison/coordinator between the bariatric facility and surgeon practice; 2) assuring compliance with clinical pathways, standard operating procedures and patient plan of care; 3) managing accreditation criteria to ensure clinic/facility/program meets expectations set forth by the governing agencies and MBSAQIP requirements; 4) providing patient/caregiver training and in-service education to staff and other stakeholders on topics relating to bariatric care; 5) coordinating the implementation of new/revised policies, standards and/or protocols; 6) providing nutritional counseling and dietary education in support of optimal patient outcomes, and 7) evaluating patient outcomes and program results to identify performance improvement opportunities.Work requires knowledge of the principles, standards, practices, protocols, terminology, tools and equipment relating to the disease of morbid obesity and the care and management of the bariatric patient. Also requires knowledge of applicable accreditation/regulatory requirements and safety/risk management standards to facilitate compliant and successful program management practices and patient outcomes.Essential Duties:* Coordiantes patient navigation through the continuum of bariatric care.* Serves as liaison/coordinator between the surgeon practice and bariatric facility; works collaboratively with interdisciplinary team to develop goals and plan interventions to maximize successful patient outcomes.* provides nutritional counseling and dietary education to facilitate healthy eating habits that support optimal patient outcomes.* Coordinates and executes the implemtation of new/revised bariatric program policies, standards, procedures and/or protocols.* Servce sa sit-based educator, coach, facilitator and resource for education/compliance program activities.* provides consultation in specialty area to all levels of healthcare providers throughout the organization, affiliated schools of nursing and the community.Education/Work Experience Requirements* Bachelor's degree Nutrition or related healthcare field, and two years of related work experience that would demonstrate attainment of the requisite job knowledge/abilities, OR* Any equivalent combination of education and experience which meets licensure requirements and provides the required knowledge, skills and abilities to perform the essential functions of the position.Licensure/Certification* Current licensure through the Academy of Nutrition and Dietetics as a Registered Dietitian is required.* Current American Heart Association Healthcare Provider BLS Certification.* National certification as a Bariatric Counselor by the American Association of Bariatric Counselors or within one year of date of hire.Additional Information* Requisition ID: 2020-R* Schedule: Full-time* Shift: Day Job* Market: CHI-Franciscan Harrison Medical Center
