As part of outreach to LTBI patients, the Patient Access Liaison connects patients with internal resources to address SDOH barriers to care. Patient Access Liaison assists patients with communication to Pharmacy to arrange medication refills when it is necessary. Patient Access Liaison assists patients with communication to providers by sending providers in-basket messages. Conduct outreach to patients with quality care gaps and assist with scheduling appointments with their primary care provider. Provide excellent customer service skills in dealing with patients, personnel, physicians, and peers.Education - High school graduate or equivalent.Experience - One year experience in clinical environment; knowledge of medical terminology and or clerical experience.Other Requirement(s) - Fluent in English and preferred bilingual in an API language.
