For detailed information on Benefits for this position, click here. The UWMC Cardiology Clinic has an outstanding opportunity for a Patient Care Coordinator Supervisor (Full-Time, Day Shifts). This position directly supervises the Heart Institute's Patient Care. This position provides PCC coverage as part of the daily operations as needed, including, but not limited to, scheduled and unscheduled absences. This position exists within our department to train, support, coach, mentor, and provide leadership to HI's Patient Care Coordinators. With the Heart Institute's overall growing volumes, this leadership structure is necessary in order to meet the needs in a structured and efficient manner.

PRIMARY RESPONSIBILITIES:

  • Assist patients, referring physicians and physician practice staff with scheduling of their Cardiac surgery, arranging complex diagnostic procedures and tests and/or entering referrals for the coordinating request for consultation among other medical specialties within the UWMC as requested by our Cardiologist. This person also works with the patient and referring physician to collect, assess, and triage information concerning the patient's medical status from the referring physicians and the community .
  • Schedule interventional cardiology proceduralists, cardiac surgeons, electrophysiologists and cardiac anesthesia is performed as needed for procedures in CVPU (Hybrid CV Procedure Suite, Cardiac Cath Labs, EP Labs), Cardiac -Thoracic-Vascular suites in the Main OR, and PACU and other settings for TEEs, Cardioversions, Biopsies, and Balloon Pump placement.
  • Supervise assigned staff ensuring they are in compliance with regulatory agencies, insurance and other financial requirements, and UWMC policies and procedures.
  • Assist staff in researching and resolving patient care issues. Receives and resolves patient complaints.
    ---Make initial contact and provides information to the patient or the referring physician on behalf of the Cardiology department as a new patient to our clinic.
  • Act as primary contact person to the patient and family, referring physician, outside agencies, staff from other hospitals and the general public by providing information and advice regarding HI Cardiovascular Ambulatory Services department.
  • Interview new patients and from that evaluation, will be able to determine which Cardiologist and/or Cardiac Surgeon, as needed, that patient should be scheduled with depending on their Cardiology needs. Provides out of town patients with housing information and UWMC shuttle services information.
  • Ensure the outcome of treatment recommendations for continued care is conveyed to referring physicians by faxing over a sheet that will provide the referring physician with the date of the scheduled appointment as well as with the name of the provider their patient will be seeing.
  • Act as a resource to physicians and fellows in the Cardiology Department.

Asks patients and families about specific beliefs, practices, and customs that may be relevant and important during medical treatment and hospitalization.   • Communication - Ability to proficiently read, write, understand, and communicate in English commensurate with the duties and responsibilities of the position to understand and respond to policies, procedures, overhead pages and alarms, in a manner that will ensure personal health and safety, and the safety of other staff, patients, and visitors, during the course of an emergency or an unusual incident, should one occur while present at the work site or in the facility.

Concerns/issues regarding departmental/organizational operations are communicated to the employee's supervisor/manager.
  • Economics of Care - Demonstrate knowledge of cost efficiencies in the delivery of care such as identifying and pursuing quality improvement opportunities and utilizing appropriate supplies and resources.

    REQUIRED QUALIFICATIONS:
    Bachelor's Degree in a medically related or public relations field AND two years of experience as a Patient Care Coordinator OR Equivalent education/experience
